General Background Information
You must give complete answers to all questions. If you answer "Yes" to any question, you must list all offenses, and for each conviction provide date of conviction and disposition, regardless of the date or location of occurrence. Conviction of a criminal offense is not a bar to employment in all cases. Each case is considered on its merits. Your answers willbe verified with appropriate police records.
Criminal Offense includes felonies, misdemeanors, summary offenses and convictions resulting from a plea of "nolo contendere" (no contest).
Conviction is an adjudication of guilt and includes determinations before a court, a district justice or a magistrate, which results in a fine, sentence or probation.
You may omit: minor traffic violations, offenses committed before your 18th birthday which were adjudicated in juvenile court or under a Youth Offender Law, and any convictions which have been expunged by a court or for which you successfully completed an Accelerated Rehabilitative Disposition program.
Professional disciplined means the annulment, revocation or suspension of your teaching or other employment certification or having received a letter of reprimand from an agency, board or commission of stategovernment, such as the Pennsylvania Professional Standards and Practices Commission.
ACT 34 COMPLIANCE (Background Check of Prospective Employees)
Each Pennsylvania resident must submit prior to employment a copy of your Criminal History Record Information report from the Pennsylvania State Police or a statement from the Pennsylvania State Police that the State Police Central Repository contains no such information relating to that person. The cost is $10.00. The criminal record history report must be no more than one (1) year old. The applicant MUST submit the ORIGINAL report prior to employment.

ACT 114 (FBI Federal Criminal History Record)
All staff, who have direct contact with children, must provide to their employer a copy of their Federal Criminal History Record that cannot be more than one (1) year old. Applicants must register (online or by phone) prior to going to the fingerprint site. The applicant will pay a fee of $28.75 for the fingerprint service and to secure the Criminal History Record. Applicants may make their payment online at www.pa.cogentid.com using a credit card or debit card. Money orders or cashiers checks payable to Cogent Systems will be accepted on site for those applicants who do not have the means to pay electronically. No cash transactions or personal checks are allowed.

ACT 151 (PA Child Abuse History Clearance)
Before a staff member can be hired, candidates must submit with his/her employment application a copy of an official clearance statement obtained from the Pennsylvania Department of Public Welfare or a statement from the Department of Public Welfare that no record exists. The clearance statement must be no more than one (1) year old. The applicant MUST submit the ORIGINAL report prior to employment.

Mission and Purpose
The mission of REYS Youth Center is to reach local youth in Christian love, to encourage, develop, and equip them to experience Godly wholeness and success by praying, sharing Christ, teaching God’s Word, and offering supervised activities.
The purpose of REYS Youth Center is to initiate and develop mentoring relationships with community youth in fulfillment of the Center’s mission statement.
Methods of Engagement
Spiritual Direction
Through our Spiritual Direction program we hope to be an example of Christ’s love through our programs, staff, and volunteers. By this effort, we strive to bring glory to God by making Christ known to all those who come and participate in our programs.
Academic Assistance
Academic Assistance programs are free for all students of the Donegal School District and surrounding areas. We focus on teaching our students the importance of homework comprehension, organization, and consistency.
Social Support
Our Social Support program’s purpose is to help the students in their everyday lives by fostering relationships, so that they can have a strong positive role in the community.
Statement of Faith
All those who participate in the on-going work and witness of REYS Youth Center attest to the the Gospel of Jesus Christ and following articles of faith:
- The Scriptures of the Old and New Testaments, being given by divine inspiration, are the Word of God, the final and supreme authority in all matters of faith and conduct.
- In the Scriptures God reveals Himself as the living and true God, Creator of all things. Perfect in love and righteous in all His ways this one God exists eternally as a Trinity of persons: the Father, the Son, and the Holy Spirit.
- God made us in His image that we might have fellowship with Him. Being estranged from God by our disobedience, we are incapable of a right relationship to God apart from divine grace.
- The only Mediator between God and us is Jesus Christ our Lord, God’s eternal Son, who as man fully shared and fulfilled our humanity in a life of perfect obedience.
- By His death in our place, Jesus revealed the divine love and upheld divine justice, removing our guilt and reconciling us to God. Having risen bodily from the dead and ascending into heaven, He rules as Lord over all and intercedes for us as our great high priest.
- The Holy Spirit, through the proclamation of the gospel, renews our hearts, persuading us to repent of our sins and confess Jesus as Lord. By the same Spirit we are led to trust in divine mercy, whereby we are forgiven all our sins, justified by faith through the merit of Christ our Savior, adopted into God’s family as His children and enabled so to live in the world that everyone may see our good works and glorify our Father who is in heaven.
- God by His Word and Spirit calls everyone into the fellowship of Christ’s body. Thus He creates the one holy, universal and apostolic church, united in the bonds of love, endowed with the gifts of the Spirit and summoned by Christ to preach the gospel and to administer the sacraments, to relieve human needs and to strive for social justice.
- God’s redemptive purpose will be consummated by the return of Christ to raise the dead, judge everyone and establish His glorious kingdom. Those who are apart from Christ shall be eternally separated from God’s presence, but the righteous shall live and reign with Him forever.
